Phase I Precertification test dates and locations are available​ in LETRS. Agencies can schedule testing via LETRS.​​

About the Physical Agility Test

• Required for POPS applicants ONLY

• Photo ID required at check-in

• Check-in time is 7:45 a.m. for a 8:00 a.m. start time

• Submit completed Form T1 – Medical Release Form

• If applicant answered YES to any of questions 1-10 on Form T1, you must submit a completed Form T1-A – Physician’s Medical Release Form.

• ALL FORMS SHOULD BE COMPLETED PRIOR TO ARRIVAL

About the Suitability Screening Test

• Required for ALL applicants

• Photo ID required at check-in

• Check-in time is 10:15 a.m. for a 10:30 a.m. start time

• Applicants must complete the on-line PHQ at the following link:  https://www.kycswcenter.com/ 
  ​Agency will not get results until this has been completed.

• Applicant does not have to bring a copy of the PHQ to the test site.​​
